SOAP notes are a portion of your pet’s veterinary medical records. They should include exam notes and lab results. SOAP stands for Subjective (your observations as the pet parent), Objective (the vet's clinical findings), Assessment (the diagnosis) and Plan (the recommended treatment or next steps).

Fetch Pet requires SOAP notes, so we can see your pet's medical history and determine what treatment is eligible for coverage under your pet insurance plan. Here’s why this matters: No pet insurance provider, including Fetch Pet, covers all pre-existing conditions. A pre-existing condition is defined as any injury, illness or condition that your pet shows signs or symptoms of either before you enroll in insurance or during the waiting period of up to 15 days.
